Within the next couple of weeks, every child that attended travel tryouts will be contacted about playing on a travel team for the upcoming fall season. Parents will be given information about the appropriate team meeting so that they may properly register their child for the upcoming travel season. This meeting is absolutely mandatory. Prior to this meeting, parents will need to download and complete the following:
1. NJ Youth Soccer Player Membership form.
2. US Youth Soccer Medical Release form, completed and notarized.
All U15 SPF Flash players must download and complete form #s 1, 2, and the US Club Medical release form.
All U8 players all NEW U9 players also need to download and complete #3. MNJYSA SAGE form.
The following teams need to download and complete #4. MCYSA SAGE form: U12 SPF Blue Pirates & U12 SPF Blue Tigers.
Parents must bring the following in order to register for the Fall 2008 season:
1. Copy of your child’s birth certificate. If your child played Travel Soccer last season, you do not need to bring a copy of your child’s birth certificate.
2. One 1" passport size photo.
Instructions:
NJ Youth Soccer Membership Form: Please fill in all personal information and have both parents/guardians and players sign the form. Do NOT complete any league or team information.
US Youth Soccer Medical Release form: This form must be completed and notarized.
Passport size photo: This photo must be a 1 inch headshot of the player and it will be used for the player pass. The photo can be either color or black and white. A Digital passport photo will be accepted as long as it’s purchased from passport photo.
Birth Certificate: To be used to register players with the State NJYS office.
SAGE Form: Please read this form and bring a signed copy to the registration meeting. There must be one form on file for each family. Both parents and child should sign the same form.
Uniform Order Form: The Travel Program will be changing the game day uniform to Under Armour uniforms tor the Fall 2008 and Spring 2009 seasons. These uniforms typically last two years. The SPFSA has all new teams and odd-numbered age groups (U8, U9, U11, & U13) purchase uniforms. If you wish to purchase a new shirt, shorts or socks, these items can be ordered the day of the registration meeting. If you are new to the SPFSA team you must purchase a uniform and this can be ordered during registration. Download and complete the order form prior to the meeting.
